Use this yearly form to register with US Roller Sports, obtaining your competative membership card.

2015 GSW Racing
Member Guidelines & Information 

This document is required for all members of GSW Racing and will be reviewed with club members at the beginning of each season. This packet contains a Membership Form to be signed and submitted to our Head Coach Troy each season.

GSW Racing GUEST session waiver form

Guests attending GSW Racing practice sessions are required to read & sign the attached form and submit it to our Head Coach Troy prior to skating.  This form is required only once per season.

GSW Racing Quad Spring Classic 2015 Entry Form

Skaters wishing to compete at the annual Quad Classic meet must sign & submit this form to Head Coach Troy in order to complete their registration.
